New patient paperwork is required for treatment.
If you are a new patient, the forms highlighted in blue will need to be returned BEFORE your first visit. You will want to keep the 1st Visit Checklist to review the night before your visit.
New Patients
How to become a new patient:
You can send by mail, email, or fax your last 3 office notes from any provider along with any imaging reports of your head or neck. As a Nurse Practitioner, I must follow the rules of the Montana Board of Nursing. These rules say that we must see patients that are within our scope of practice. After I review these notes, you will be contacted regarding if we will be able to proceed.
